- They must have an original valid Party City receipt consisting of a survey code.
- Their visit to the store should not be long ago.
- Must be appropriate age and fulfilled all the requirements mentioned in the receipt.
Instructions
- Visit the Party City Survey page at www.partycityfeedback.com
- Enter the Time of Visit from your Party City purchase receipt.
- Then enter the 14-digit survey code from that receipt.
- Click the “Start” button to begin the survey.
If you need help finding the information on your receipt, take a look at the sample receipt image highlighting the numbers posted the right side of the survey page.
In the survey, you will be asked a few questions related to the products, services as well as your experience with Party City. Answer them to the best of your knowledge and follow the instructions on-screen to redeem your special prize.
