• The participants will be taken to another page where the required info has to be entered
• Please enter the first 4 digits of the store number

• The entrants may refer to the sample image given above the blank field
• Then accomplish the subsequent steps and finish the survey.
• Once the survey is finished, the entrant will be eligible for one entry for the monthly lucky draw
